Job summary

BAYADA at Inspira Home Health is seeking an experienced Licensed Practical Nurse (LPN) for a part-time opportunity providing in-home visits in Gloucester County, NJ. The LPN will join a multidisciplinary team delivering skilled nursing and rehabilitative care to help clients stay at home.

Requirements include a current NJ LPN license and at least one year of LPN experience; home care experience preferred. Travel to cases is required.

Qualifications

• Active LPN license in New Jersey.
• Minimum of one year of verifiable LPN experience.
• Graduation from an accredited nursing program.
• Home care experience preferred, not required.
• Ability to travel to cases and manage time independently.
• Strong interpersonal and computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to managers.
• Follow doctors' orders and document observations in EMR.
• Perform client assessments and participate in case management.
• Document care activities accurately on a state-of-the-art tablet.

Job description

BAYADA at Inspira Home Health is currently seeking an experienced Licensed Practical Nurse, for a part-time opportunity performing home health visits for our Gloucester County office, located in New Jersey. This office services clients on a per visit basis in territories throughout Gloucester County.

As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Licensed Practical Nurse (LPN) Responsibilities:

• Making home visits to clients living in designated territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures.
• Monitoring clients' conditions; reporting changes to Clinical Manager and Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a state-of-the-art touch pad tablet.

Qualifications of a Licensed Practical Nurse (LPN):

• A current license as a Licensed Practice Nurse in the state of New Jersey.
• A minimum of one year of recent, verifiable experience as an LPN.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Prior home care experience strongly preferred, but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Additional Information:

Base Pay, depending upon qualifications, status, and subject to negotiation: $35.00-$42.00 per point
