Job summary

Lutheran SeniorLife is seeking a Licensed Practical Nurse LPN to provide direct patient care in home settings across western Pennsylvania. The role involves in-home visits, coordinating with RNs, and supporting hospice and palliative services.

Full-time shift 8:00 a.m. to 4:30 p.m. with rotating weekends; eligibility for sign-on bonus; benefits include health coverage, PTO, retirement plan.

Qualifications

• Currently licensed as a Licensed Practical Nurse in Pennsylvania.
• One year of acute care clinical or commensurate experience preferred.
• Home health care or a related appropriate experience preferred.
• Must have dependable transportation with a valid driver’s license.
• Working knowledge of computers, Microsoft Word and Excel required

Responsibilities

• Provide direct patient care and nursing services under RN supervision.
• Visit patients in their homes and coordinate with the care team.
• Support in-home palliative and hospice services as assigned.

We are offering a $6,000 Sign-On Bonus (to be paid throughout the first year of employment) for the Licensed Practical Nurse LPN.

Rates starting at $27/ hour.

Full-time, 8:00 a.m. to 4:30 p.m., rotating weekends
