Job summary

A leading hospice care provider is seeking a compassionate Hospice LPN to deliver skilled nursing care in patients’ homes. In this role, you will work with an interdisciplinary team to provide holistic support to families during difficult times. The successful candidate will have an unrestricted LPN license in New Mexico, a minimum of one year of nursing experience, and strong communication skills. Qualifications

About the Role

As a Hospice LPN, you’ll provide skilled, compassionate nursing care in patients’ homes while supporting families through one of the hardest seasons they will ever face. Key Responsibilities

  • Provide skilled hospice nursing care under the direction of a Registered Nurse and the established plan of care
  • Administer medications and treatments in accordance with physician orders and agency policies
  • Monitor and document changes in patients’ physical, emotional, and psychosocial status, reporting significant findings promptly
  • Educate patients and caregivers on comfort measures, medications, and what to expect throughout the hospice journey
  • Collaborate closely with RNs, social workers, aides, chaplains, and other team members to ensure coordinated, patient-centered care
  • Maintain accurate, timely clinical documentation in the EMR in compliance with CareM standards
  • Participate in interdisciplinary team meetings, case conferences, and quality improvement activities
  • Navigate complex family dynamics with professionalism, empathy, and healthy boundaries

Requirements

What We’re Looking For
  • Graduate of an accredited practical or vocational nursing program
  • Current, unrestricted LPN license in the state of New Mexico
