Job summary

Help At Home in Ohio is seeking a Licensed Practical Nurse (LPN) – Home Visits to deliver skilled nursing care in clients' homes, following physician orders and the established medical plan. This role emphasizes compassionate care, accurate documentation, and adherence to CHC and agency safety protocols; candidates must have an Ohio LPN license and CPR/First Aid certification.

Preferred candidates have at least one year of experience caring for medically fragile individuals and can coordinate

  • One year of experience providing skilled care to the medically fragile.

Responsibilities

  • Implements the medical plan of treatment and follows physicians' orders.
  • Monitors client health and safety in accordance with CHC and agency protocols.
  • Documents activities and services accurately and timely according to CHC and agency regulations.
  • Monitors general health conditions and notifies supervisor if treatment is needed.

Education

Ohio LPN license

Responsibilities
  • Implements the medical plan of treatment and follows physicians' orders.
  • Monitors individual at all times, following all CHC and agency safety protocols.
  • Documents activities and services accurately and timely according to CHC and agency regulations.
  • Monitors general health conditions of the individual and notifies supervisor of any medical issues or if treatment is needed.

Qualifications
  • Ability to work in a professional, ethical, and respectful manner.
  • Ability to provide personal care for non-ambulatory clients, use a Hoyer lift, maneuver wheelchairs, and transport clients to community events and social activities.
  • Strong communication skills, oral and written; strong interpersonal skills.
  • Ability to anticipate and resolve problems demonstrating good judgment.
Education And Experience
  • Current Ohio LPN license
  • Current CPR/First Aid certification.
  • One year of experience providing skilled care to the medically fragile.
