Job summary

Enhabit Home Health & Hospice is seeking an experienced Licensed Practical Nurse to provide in-home nursing care under the direction of registered nurses and physicians. This role requires a valid state license and reliable transportation to visit patients across homes and communities.

The ideal candidate delivers compassionate patient care, adheres to care plans, and maintains accurate documentation while navigating the realities of home-based settings.

Qualifications

• Must possess a valid state driver's license.
• Must possess automobile liability insurance.
• Must wear seat belts at all times while driving.
• Dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed as a Licensed Practical Nurse in the State of employment.

Responsibilities

• As a LPN, you will visit patients in their homes and provide nursing care for them under direction of RNs and in compliance with the physicians' orders.
