Job summary

Enhabit Home Health & Hospice is seeking a Licensed Practical Nurse to provide in-home nursing care under the direction of registered nurses and in line with physician orders. This full-time role offers the chance to impact patients’ daily lives in a supportive, team-based environment.

You will drive to patient homes, maintain accurate care documentation, and collaborate with a coordinated care team to deliver high-quality, respectful care.

Qualifications

• Must possess an active LPN license in the state of employment.
• Must have valid state driver’s license and automobile liability insurance.
• Must be able to drive to patients’ homes and work in various weather conditions.

Responsibilities

As a LPN, you will visit patients in their homes and provide nursing care for them under direction of RNs and in compliance with the physicians' orders.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seat belts at all times while driving.
• Dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed as a Licensed Practical Nurse in the State of employment.
