Job summary

Enhabit Home Health & Hospice in Kentucky is seeking a dedicated Licensed Practical Nurse to join our home-based care team. You will provide nursing services to patients in their homes under the direction of RNs and in line with physician orders.

The role requires a valid LPN license in the state, a reliable vehicle, and a commitment to compassionate, patient-centered care across communities served by Enhabit.

Qualifications

• Must hold a valid LPN license in the state of employment.
• Must possess a valid driver’s license and automobile insurance.
• Reliable transportation and ability to drive in all weather conditions.

Responsibilities

As a LPN, you will visit patients in their homes and provide nursing care for them under direction of RNs and in compliance with the physicians' orders.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seat belts at all times while driving.
• Dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed as a Licensed Practical Nurse in the State of employment.
