Responsibilities

• Perform selected nursing procedures within the scope of LPN practice
• Provide patient and family teaching as outlined by the RN
• Monitor vital signs and record observations
• Monitor and record intake and output; report significant changes immediately
• Observe, assess, and document patient's physical and mental condition; report any changes
• Administer medications as instructed by the physician and RN; document accordingly
• Participate in multidisciplinary patient case conferences, staff meetings, and in-service education
• Cooperate with on-call schedules, including weekends and after-hours visits

Qualifications

• Valid LPN license in good standing

• Preferred experience in home health, hospice, hospital, or skilled nursing

• Active CPR certification

• Familiarity with electronic health records

• Strong problem-solving and critical-thinking skills

• Ability to thrive in a fast-paced environment

• Reliable transportation, valid driver’s license, and current auto insurance
