Responsibilities

• Perform nursing procedures within the scope of LPN practice
• Provide patient and family education as outlined by the RN
• Monitor vital signs and record measurements and observations
• Track and document intake and output, reporting significant changes immediately
• Assess and record patient's physical and mental condition, reporting any changes
• Administer medications as ordered by the physician and instructed by the RN, documenting appropriately
• Participate in multidisciplinary patient case conferences, staff meetings, and in-service education
• Assist with on-call schedule, including weekends and after-hours visits

Qualifications

• Licensed Practical Nurse (LPN) license in good standing
• Previous experience in home health, hospice, hospital, or skilled nursing preferred
• Active CPR certification
• Familiarity with electronic health records
• Strong problem-solving and critical-thinking skills
• Ability to work effectively in a fast-paced environment
• Reliable transportation, valid driver's license, and current automobile insurance
