BAYADA Home Health Care

is seeking a compassionate and dedicated Licensed Practical Nurse, LPN to join our home health visits team. This role involves working with adult and senior clients on a per-visit basis in homes throughout Union County, Somerset County, Middlesex County, and Monmouth County.

This is a full-time opportunity. A minimum of one year of prior clinical experience as a Licensed Practical Nurse is required. As a home care nurse, you will be an integral part of a multi-disciplinary healthcare team providing skilled nursing and rehabilitative care, enabling clients to receive necessary medical care at home.

Responsibilities of a Licensed Practical Nurse (LPN):

• Making home visits to clients in designated areas
• Administering medication, wound care, treatments, and procedures
• Monitoring client conditions and reporting changes
• Following and documenting doctors' orders accurately
• Performing client assessments as needed
• Case management and coordination
• Documenting observations, interventions, and evaluations using electronic medical records on a tablet

Qualifications:

• Current NJ Licensed Practical Nurse license
• At least one year of recent, verifiable LPN experience
• Graduation from an accredited nursing program
• Home care experience is preferred but not required
• Ability to work independently and manage time effectively
• Strong interpersonal skills
• Solid computer skills; EMR experience preferred
• Ability to travel as needed
