Job summary

AccentCare in Forest, MS is seeking a Licensed Vocational Nurse/LPN for a full-time role. Under RN supervision, you will provide direct patient care across pediatric to geriatric patients and document progress.

The position is on-site with a typical schedule Monday-Friday 8:00am-4:30pm and occasional on-call visits. Valid LPN/LVN license and CPR certification are required; driving to multiple locations may be needed.

Qualifications

• Graduate of an accredited practical or vocational nursing program
• Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation
• One year of LVN/LPN work experience
• Ability to provide nursing visits according to the treatment as prescribed by the physician and provide and maintain paperwork to maintain accurate and complete medical records
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver who can travel to all business locations

Responsibilities

• Performs technical nursing functions and direct patient care consistent with the established plan of care and under the supervision of an RN.
• Promotes and maintain optimal client care under the supervision of an RN.
• Maintains a safe patient environment.
• Identifies and communicates significant abnormalities in patients data to case manager and other appropriate home health team members.
• Administers prescribed medications and treatments as permitted by law. Assesses all medicines a client may be taking to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, contraindicated medications and reports any problems to the physician.
• Re-evaluates client's nursing needs and reports findings to Case Manager.
• Counsels and instructs the client and family in meeting nursing and related needs.
• Prepares clinical and progress notes.
• Submits all documentation in a timely manner in accordance with agency policy.
• Documentation on patients chart correlates with nursing care plan.
• Communicates frequently and works closely with other home care team members.
• Assists in coordinating services provided to client

On Call: LPNs are asked to help occasionally to assist with standard visits if on call primary and backup are too full on Saturdays.

The LVN/LPN observes and documents the patient's medical progress and nursing status changes under the direction of a registered nurse.
