Schedule: Full Time

Shift: Monday - Friday, 8am to 5pm

Location: Travel across Buncombe, Henderson, Transylvania, and Polk counties

Role Overview:

Provide direct patient care to home health patients in their homes, addressing psychosocial, physical, communication, and educational needs as outlined in the Plan of Care. Work under the supervision of the RN case manager or RN Patient Care Supervisor, adhering to regulatory and agency standards. Communicate effectively with team members regarding patient conditions and diagnostic results.

Key Responsibilities:

• Follow home health policies, procedures, and care standards
• Deliver routine patient care under supervision
• Observe, document, and report patient reactions and condition changes
• Maintain accurate and detailed care documentation
• Maintain competency in essential nursing skills such as wound care, infusion therapy, and medication oversight

Qualifications:

• At least 2 years of recent acute care experience as an LPN
• At least 1 year of recent home health care experience
• Computer proficiency
• Effective communication skills in English
• State licensure as an LPN upon hire
• Active American Heart Association BLS certification
• Valid driver’s license in the state of employment upon hire

Preferred Qualifications:

• Experience with Point of Care software systems
• Additional language skills
