As a home health nurse, you will be an integral member of a multi-disciplinary healthcare team that provides skilled nursing and rehabilitative care to clients, allowing them to remain at home.

Licensed Practical Nurse (LPN) Responsibilities:

• Making home visits to clients in designated areas.
• Administering medication, wound care, treatments, and procedures.
• Monitoring client conditions and reporting changes.
• Following doctors' orders and documenting appropriately.
• Performing client assessments as needed.
• Case management and coordination.
• Documenting observations, interventions, and evaluations using electronic medical records on a tablet.

Qualifications of a Licensed Practical Nurse (LPN):

• Current license as a Licensed Practice Nurse in MD.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.
