Schedule: Full time

Location: This position will conduct patient home health visits in the Hernando County area.

The role you’ll contribute:

Administer nursing care to patients according to the physician’s order and under the supervision of a Registered Nurse. Provide intermittent, individualized nursing care, teach and supervise patients and family members or caregivers to ensure continuity of care and assist in implementing comprehensive care plans. Deliver high-quality nursing care, perform technical nursing skills proficiently, and promote a team approach in patient care. Execute physician’s orders and notify RN/Case Manager of any updates or changes in patient condition. Complete documentation per agency policy and regulatory guidelines, and perform home visits as ordered. Continued employment depends on your ability to perform these duties effectively.

  • Comply with all department policies, procedures, practices, and standards of care.
  • Provide direct patient care responsibilities efficiently and organized, under the supervision of the RN.
  • Perform treatments within scope of training and administer medications to assigned patients.
  • Collect data specific to the patient’s age and contribute to their care plan.
  • Report pertinent observations to the attending physician and RN, and implement changes in the care plan as directed. Communicate these changes to the home health team.
  • Verify and implement physician orders (orders taken by the LPN are co-signed by an RN).
  • Demonstrate effective communication skills with team members, patients, and families.
  • Maintain clinical records according to standards and within required time frames.

The expertise and experience you’ll need to succeed:

  • High School diploma or equivalent
  • Vocational training from an accredited school of Licensed Practical Nursing
  • Current state LPN licensure
  • Active American Heart Association BLS certification
  • Valid driver’s license with current car insurance in the state
