Home Health Care Inc. is seeking a Licensed Practical Nurse (LPN) to join our Home Health team in the Twin Cities Metro Area. This full-time role offers a competitive per-visit compensation and full benefits.

The ideal candidate is a MN-licensed LPN with experience in home health, hospice, or similar settings, capable of working autonomously and collaborating with RN Case Managers and the care team.

Qualifications

• Licensed as an LPN in Minnesota.
• Reliable vehicle with current insurance and valid Driver’s License.
• Experience in TCU, Hospice, or Home Health preferred.

Responsibilities

Job Title: Licensed Practical Nurse - LPN Visit Nurse

Location: Twin Cities Metro Area

Schedule: Full-Time Availability

Compensation: $30 - $38 per visit

Position Summary

Home Care is in high demand and we are looking for experienced and caring LPNs to join our team at Home Health Care Inc. Successful applicants will have a proven track record as an LPN. Home care experience is a huge benefit but not required.

Responsibilities

• Provide patient care according to client’s care plan and needs.
• Perform wound care treatments and assist clients with medication management.
• Teach the client and/or family members about care and support skills.
• Communicate regularly with RN Case Managers, Supervisors, and the Care Team.

Qualifications

• Licensed as a Practical Nurse / LPN in the state of Minnesota.
• Driving is required; must have a reliable vehicle with current insurance and a valid Driver’s License.
• Demonstrated ability to work autonomously.
• Prior nursing experience in a TCU, Hospice, or Home Health setting.
• Experience with Electronic Medical Records (EMR) software is preferred.
• Military Occupational Classification (MOC) Codes: Army – 68C; Navy – HM; Air Force – 4N0X1; Marines – HM; Coast Guard – HS.
