Job summary

Home Health Care Inc. is seeking licensed practical nurses (LPN) to join our visit-nurse team in the Twin Cities Metro Area. This full-time role offers meaningful patient care, wound care, medication assistance, and education to clients and families per care plans.

You will collaborate with RN case managers and the care team to ensure high-quality home health services. Applicants must hold Minnesota LPN licensure, have a valid driver's license and reliable vehicle, and be capable of working

Qualifications

• Must be licensed as an LPN in Minnesota.
• Ability to work autonomously and manage responsibilities.
• Valid driver’s license and reliable vehicle required.
• Experience in home health, hospice, or TCU preferred.

Responsibilities

Job Title: Licensed Practical Nurse - LPN Visit Nurse

Location: Twin Cities Metro Area

Schedule: Full-Time Availability

Compensation: $30 - $38 per visit

Position Summary

Home Care is in high demand and we are looking for experienced and caring LPNs to join our team at Home Health Care Inc. Successful applicants will have a proven track record as an LPN. Home care experience is a huge benefit but not required.

Responsibilities

• Provide patient care according to client’s care plan and needs.
• Perform wound care treatments and assist clients with medication management.
• Teach the client and/or family members about care and support skills.
• Communicate regularly with RN Case Managers, Supervisors, and the Care Team.

Qualifications

• Licensed as a Practical Nurse / LPN in the state of Minnesota.
• Driving is required; must have a reliable vehicle with current insurance and a valid Driver’s License.
• Demonstrated ability to work autonomously.
• Prior nursing experience in a TCU, Hospice, or Home Health setting.
• Experience with Electronic Medical Records (EMR) software is preferred.
• Military Occupational Classification (MOC) Codes: Army – 68C; Navy – HM; Air Force – 4N0X1; Marines – HM; Coast Guard – HS.
