Location: Northeast Philadelphia, PA 19149

Setting: One-on-One Home Care

Penn Home Health Care is seeking a dependable and compassionate Licensed Practical Nurse (LPN) to provide skilled nursing services to a participant in the Northeast Philadelphia (19149) area.

This is a one-on-one home care assignment focused primarily on wound care, medication management, ongoing assessment, and coordination of the participant’s skilled nursing needs.

Responsibilities

• Perform wound assessments and provide wound care as ordered
• Administer and manage medications in accordance with physician orders and the plan of care
• Monitor the participant’s condition and identify/report changes
• Complete accurate and timely skilled nursing documentation
• Follow infection-control and safety procedures
• Communicate with the RN, physician, service coordinator, and other members of the care team as appropriate
• Provide participant education related to medications, wound care, and the plan of care
• Follow all Penn Home Health Care policies and applicable state and payer requirements

Qualifications

• Current, unrestricted Pennsylvania LPN license
• Current CPR/BLS certification
• Prior wound-care experience preferred
• Experience with medication management
• Home care, home health, private-duty nursing, or community-based nursing experience preferred
• Ability to work independently in a home setting
• Reliable transportation
• Must successfully complete all required employment screenings, clearances, and credentialing requirements
