Home Health Care, Inc. in Minnesota is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing visits in patients' homes, delivering compassionate, patient-centered care with time for meaningful interactions.

You'll perform wound care, administer medications, reinforce education from the RN Case Manager, monitor conditions, and document accurately while collaborating with a multidisciplinary team.

Qualifications

• Active Minnesota Licensed Practical Nurse (LPN) license.
• Reliable transportation with valid driver's license and current insurance.
• Ability to work independently and manage daily schedule.
• Previous nursing experience in home health, skilled nursing, rehab, or similar.
• Strong communication skills and patient-centered care.
