Job summary

Emerson Health, Concord, MA, is seeking a dedicated Licensed Practical Nurse (LPN) to join our Home Care team. You will provide direct nursing services to patients in home and community settings, helping them maintain independence and achieve optimal health outcomes.

Under supervision of the Nursing Supervisor, you will assess patients, implement care plans, and collaborate with primary nurses to ensure patient-centered care while maintaining accurate documentation and privacy standards.

Qualifications

• Graduate of an approved Practical or Vocational Nursing program.
• Minimum of two years of medical or surgical nursing experience strongly preferred.
• Home care or home health experience preferred.

Pay Range: $29.00 - $37.00 / Hourly

Shift: Day/Eve shift

Hours: approx 8:00am-4:30pm

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Home Care team. In this role, you will provide direct nursing care to individuals and families in their homes and community settings, helping patients maintain independence and achieve the best possible health outcomes.

Working under the guidance of the Nursing Supervisor and in collaboration with the patient's primary nurse, you will play a vital role in delivering patient-centered care, monitoring health conditions, and supporting patients and their families through compassionate service.

Minimum Qualifications

Education

Experience

• Knowledge and experience in community health nursing preferred.


• Home care or home health experience preferred.

Licensure and Certification

• Current Massachusetts Licensed Practical Nurse license.


• Basic Life Support certification for Healthcare Providers required.
