Job Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. In this role, you will provide essential nursing care to patients in various settings, including hospitals, clinics, and long-term care facilities. You will work under the supervision of registered nurses and physicians, delivering high-quality care that meets the physical, mental, and emotional needs of our patients.

Responsibilities

• Provide direct patient care including monitoring vital signs and assessing patient conditions.
• Administer medications and treatments as prescribed by registered nurses or physicians.
• Assist in the development and updating of patient care plans based on individual needs.
• Maintain accurate patient records and documentation to ensure continuity of care.
• Support patients and their families with emotional and educational support regarding health conditions.
• Collaborate with healthcare team members to optimize patient outcomes and discharge planning.
• Ensure a clean and safe environment, adhering to infection control practices and safety standards.

Requirements

• Graduate of an accredited school of professional nursing.
• Current license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in the state(s) of employment.
• Minimum of one (1) year experience in an acute care setting or equivalent experience.
• CPR certified.
• Demonstrated written, verbal, and interpersonal communication skills.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Has a good driving record.
