Preferred Care Home Health Services is seeking experienced Licensed Practical Nurses (LPNs) to join our home health team in Fort Myers, FL. This full-time role offers compassionate, patient-centered care delivered in patients' homes under the supervision of a Registered Nurse.

Qualifications

• Current and unrestricted Florida LPN license.
• Graduate of an approved practical nursing program.
• Previous home health experience preferred.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and appropriate auto insurance.
• Strong clinical assessment, communication, organization, and documentation skills.
• Ability to work independently while collaborating effectively with an interdisciplinary care team.
• Compassionate, professional, and patient-centered approach to care.

Responsabilités

• Provide skilled nursing services per physician’s plan of care in the patient’s home.
• Nursing care under direction of a Registered Nurse.
• Administer medications and treatments as ordered.
• Monitor patient condition and share significant findings with RN/physician.
• Document patient care and clinical notes accurately.
• Maintain timely clinical notes per policy and regulations.
• Participate in case conferences, reviews, and continuing education.
• Assist in developing and evaluating the patient’s plan of care with interdisciplinary team.
• Educate patient and caregiver on medications, treatments, and care techniques.
• Communicate with physicians and other healthcare professionals.
• Provide education and support to patient, caregiver, and family.
• Support QA/QI initiatives within the agency.
• Adhere to agency policies, procedures, and Code of Conduct.

Position Summary

The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their homes according to the physician’s plan of care and under the direction and supervision of a Registered Nurse.

Essential Duties and Responsibilities

• Provide skilled nursing services in accordance with the physician’s plan of care and home health certification.
• Provide nursing care under the direction and supervision of a Registered Nurse.
• Administer medications and treatments as ordered.
• Monitor patients for changes in condition and promptly communicate significant findings to the supervising RN and physician.
• Accurately document patient care, clinical findings, treatments, and interventions in the medical record.
• Prepare timely and complete clinical notes in accordance with agency and regulatory requirements.
• Participate in case conferences, utilization reviews, orientations, in-services, and continuing education.
• Assist with the development and evaluation of the patient’s plan of care in collaboration with the patient, caregiver, representative, and interdisciplinary team.
• Provide patient and caregiver education regarding medications, treatments, disease processes, and appropriate care techniques.
• Communicate effectively with physicians and other healthcare professionals involved in the patient’s care.
• Provide patient, caregiver, and family support and education as appropriate.
• Participate in the agency’s Quality Assurance and Performance Improvement (QAPI) program.
• Maintain compliance with agency policies, procedures, regulatory requirements, and Code of Conduct.
• Perform other duties as assigned.
