Licensed Practical Nurse (LPN) - Full-Time, Day Shift

Job Summary

The Licensed Practical Nurse (LPN) performs nursing functions within the scope of practice and provides and/or coordinates care and services to meet the residents' physical, psychosocial, and functional care needs.

Job Responsibilities
  • Practice nursing care within the scope of licensure and in accordance with the state Nurse Practice Act; keep a current active license; follow state and federal licensure rules and regulations, as well as EverTrue policies and procedures.
  • Collaborate with the interdisciplinary team to define individualized care plans; work with nursing leadership to ensure regulatory compliance; promote learning and provide role modeling for staff.
  • Administer medications, oversee medication processes, and monitor administration of IV fluids, treatments, and medications as ordered.
  • Initiate IV therapy if IV-certified.
  • Maintain knowledge of observation, assessment, and documentation processes, including care plans and the MDS as applicable.
  • Use electronic medical records to document clinical information accurately and timely.
  • Observe resident status, coordinate care with other services, and document medical information thoroughly.
  • Ensure a safe, comfortable environment by assessing hazards and implementing safety practices.
  • Supervise and direct certified staff and others to ensure standards of care are met.
  • Report and support investigations related to staff or resident incidents, in compliance with regulations.
Qualifications, Knowledge, Skills & Abilities
  • LPN licensure in the practice state.
  • CPR certification required.
  • Specialized training or experience in gerontological nursing preferred.
Physical Requirements and Working Conditions

Must be able to lift 30 lbs, push/pull 50 lbs, and perform physical activities such as standing, walking, and handling objects. Must be able to work beyond normal hours, including weekends and holidays, and be exposed to various environmental conditions and potential hazards, following safety protocols.

EverTrue Pay & Benefits

Pay grade: 12, $23.55/hour to $37.75/hour, with shift differentials available.
