Position Overview

Patriots Colony is hiring Full-time LPNs and offering a sign-on bonus of $4,000!

Works under the supervision of a Registered Nurse, Nurse Manager, or Nursing Supervisor, and is ultimately responsible to the Director of Nursing or Assistant Director of Nursing and Director of Quality Care Management. The LPN participates in developing, maintaining, or modifying nursing care plans. Provides emotional, physical comfort, and safety for residents and their families. Assists in supervising auxiliary personnel.

Responsibilities
  1. Administer medications and treatments accurately according to provider’s orders.
  2. Respect resident choices within safety and dignity limits, adhering to facility policies on privacy and dignity.
  3. Assign duties to non-professional staff such as CNAs based on their education, training, and resident needs to ensure quality care. Engage team members for optimal performance and address performance issues as needed.
  4. Contribute to the development, maintenance, or modification of individual service plans. Recognize changes in condition with proper documentation according to nursing standards. Follow through to ensure positive resident outcomes using good judgment and specialized skills.
Requirements
  1. LPN Diploma from an accredited program (required for new grads within 90 days of completion).
  2. CPR/BLS certification (within 30 days of hire) and FA certification (within 30 days of hire).
  3. LPN license in VA or a compact state, or board eligibility.
