#LI-SA1 Licensed Practical Nurse - Hospice Full-time in Central WI & surrounding areas $29-35/hr, based on experience

Summary/Objective:

The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the team to meet positive patient care outcomes.

Essential Functions:

  • Provides direct patient care as defined in the State Nurse Practice Act, including specialized duties related to end-of-life care.
  • Implements current nursing practices following comprehensive assessment and the plan of care.
  • Provides accurate and timely documentation of patient services to reflect the plan of care.
  • Assesses and provides patient and family/caregiver education pertinent to diagnosis and self-care plan.
  • Participates in coordinating hospice services, reporting needs to the interdisciplinary team, including hospice aides, OT, PT, SW, ST, Dietician, or Clinical Supervisor.
  • Uses and prepares equipment and supplies effectively, adhering to aseptic techniques.
  • Provides appropriate pain and symptom management, evaluating patient responses to treatments/medications.
  • Participates in personal and professional growth, as well as in the organization's quality assessment and performance improvement activities.
Work Environment/Physical Requirements:

The position involves significant physical activities including standing, lifting, bending, stooping, pushing, pulling, and twisting. The employee must be a licensed driver with a well-maintained and insured vehicle. Work conditions may vary due to weather and travel needs, requiring adaptability.

Required Education and Experience:
  • Licensed Practical Nurse graduated from an accredited program, currently licensed and in good standing with the Wisconsin State Board of Nursing.
Preferred Education and Experience:
  • One to two years of nursing experience in an acute care setting and one year in home health, hospice, or similar.
  • Interest in hospice philosophy and comfort care.
  • Ability to work cooperatively with multidisciplinary teams including social workers, chaplains, CNAs, and volunteers.
