As an LPN, you will provide patient-centered care, administer medications, follow care plans, document accurately, and collaborate with an interdisciplinary team to support residents' well-being.

Qualifications

• Current, unencumbered LPN license in MA.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care per standard nursing practice and plan of care.
• Follow and implement individualized care plans from Nurse Manager and team.
• Perform medication administration and prescribed treatments per care plan.
• Monitor residents’ conditions and report changes to supervising nurse or physician.
• Maintain accurate, timely documentation per orders and regulatory standards.
• Support a safe, respectful, therapeutic environment for residents and families.
