We are always looking for qualified Licensed Practical Nurses - LPN to join our team!

Location : Wellington Rehabilitation and Healthcare

Job Type : Full-Time, Part-Time, PRN

Are you a compassionate Licensed Practical Nurse (LPN) looking to provide high-quality care in a warm and supportive environment? Join our dedicated team and play a vital role in improving the well-being and quality of life of our residents.

Major Responsibilities

• Provide direct patient care to residents and patients of the health and rehabilitation center (skilled nursing home).
• Conduct and document a thorough evaluation of each resident's medical status upon admission and throughout the resident's course of treatment.
• Assist in the implementation of individualized treatment plans for each assigned resident.
• Oversee Falls Program and Pressure Sore Program.
• Supervises staff and oversees patient care in accordance with standards of care and practice and applicable state / federal regulations.
• Follows established facility policies and procedures while providing and coordinating resident care with other facility personnel.
• Supervises Certified Nursing Assistants during shifts and participate in their evaluation.

Minimum Qualifications

• Must be a Licensed Practical Nurse (LPN) with an active, unencumbered state license.
• Previous experience in long-term care, skilled nursing, or geriatric nursing preferred.
• Strong clinical skills, excellent communication, and a passion for patient-centered care.
• Ability to multitask, work in a fast-paced environment, and adapt to changing resident needs.
