Job summary

Wilkinson Residential Health Care Facility is seeking an Licensed Practical Nurse (LPN) to join our Amsterdam, NY team for evening shifts. You will provide direct patient care, administer medications, monitor residents, and collaborate with a multidisciplinary team.

Qualified candidates are NY-licensed LPNs with English proficiency, BLS certification within 90 days, and the ability to work evenings and weekends while delivering compassionate care and accurate EMR documentation.

Qualifications

• Must hold an active LPN license in New York
• English language proficiency required (read, speak, follow instructions)
• BLS certification obtained or within 3 months of hire
• Able to work evenings and weekends
• Strong communication and interpersonal skills
• Basic computer skills and familiarity with EMR systems
• Compassionate and patient-centered care approach

Responsibilities

• Administer medications, wound care, and vital sign checks for residents
• Educate residents and families about treatment plans and post-discharge care
• Monitor resident conditions and report changes to nursing team/physicians
• Assist with transfers, mobility, and ambulation
• Identify safety hazards and take corrective action to protect residents
• Maintain accurate records of resident care in EMR and charts
• Contribute to quality improvement initiatives to improve outcomes
• Collaborate with other departments to ensure seamless care delivery

Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and vital sign checks.
• Educate patients and families about disease treatment plan, including self-care post discharge.
• Monitor and report changes in resident condition to the nursing team and attending physicians.
• Assist with resident transfers, ambulation, and mobility exercises.
• Assures resident safety by identifying safety hazards, emergency situations and initiating corrective action as appropriate.
• Maintain accurate and up-to-date records of resident care and treatment.
• Participate in quality improvement initiatives to promote excellent patient outcomes.
• Collaborate with other departments to ensure seamless delivery of care.

Requirements:

• Currently licensed as a Licensed Practical Nurse (LPN) in the State of New York.
• Must be able to speak, read and follow instructions in English.
• BLS Provider obtained with within 3 Months (90 days) of hire date or Job transfer date.
• Ability to work variable shifts, including evenings and weekends.
• Strong communication and interpersonal skills.
• Basic computer skills and familiarity with electronic medical records.
