Job summary

Lutheran Care Center at Concord Village is seeking a Licensed Practical Nurse (LPN) on a per diem basis to support multiple shifts at our New York facility. The role emphasizes resident care planning, supervision of CNAs, and adherence to state and federal nursing regulations.

Responsibilities include coordinating with interdisciplinary teams, admitting and discharging residents, and maintaining thorough charting. Vaccination requirements apply; CPR and NYS LPN license are required.

Qualifications

• One year certificate from college or technical school for Nursing is required.
• Minimum one year of long term care or acute care experience is required.
• Valid NYS LPN license and CPR certification are required.
• Facility requires all employees to be fully vaccinated for COVID-19, MMR, and PPD.

Responsibilities

• Ensure daily licensed nursing functions comply with state and federal regulations.
• Supervise CNA delivery of care and report trends to the Nursing Supervisor.
• Admit, transfer, and discharge residents.
• Charting, documentation, and care planning.
• Coordinate shift services with interdisciplinary team.
• Monitor seriously ill residents as needed.
• Maintain safety and infection control practices.

SAT-SUN Day $34.68/hr (base+2%), Evening $37.40/hr (base+10%), $38.76/hr(base+14%)

Summary:

The Licensed Nurse is responsible for managing the residents existing care plans and supervising the day-to-day resident care activities during the shift. Both care management and supervisory responsibilities must be executed in accordance with state and federal regulations and facility policies and procedures.

Responsibilities include the following.

• Ensure that daily licensed nursing functions are carried out in accordance with state and federal regulations,
• and facility policies and procedures.
• Cooperate with other departments in the facility when coordinating resident care services to ensure that residents’ total regimen is maintained.
• Report resident needs to next shift RN Supervisor .
• Supervise the CNA’s delivery of care and proper implementation of the standard of care. Report any ongoing trends to the Clinical Nurse Manager/Nursing Supervisor.
• Complete medical forms, reports, evaluations, studies, charting, care plans etc. as needed. Attends care plan meetings as needed.
• Meet with interdisciplinary team members as needed to plan and coordinate shift services and programs.

Supervisory Responsibilities

Directly supervise Certified Nursing Assistants employees in accordance with the organization's policies and all applicable laws. Plan, assign, and direct work, as well as address complaints and resolve problems.

Education and/or Experience

• One year certificate from college or technical school for Nursing required.
• Minimum one year of long term care or acute care experience required.
• Valid NYS LPN license and CPR required.
