Facility Name and Address:1890 SR 436, suite 355, Winter Park, FL 32792

Purpose:

Provides effective and efficient patient care under the supervision of a registered nurse and in accordance with the patient’s plan of care, applicable law and regulation, accepted standards of nursing practice, payor requirements and Interim HealthCare protocols and procedures.

  • Participates in assessments: Actively participates in ongoing interdisciplinary patient assessments.
  • Collaborates on care plans: Works with patients, caregivers, and RN supervisors to develop and evaluate the care plan.
  • Provides physician-ordered services: Delivers services as outlined in the patient’s care plan.
  • Monitors patient condition: Identifies and reports any changes in the patient’s condition to the RN supervisor.
  • Counsels and educates: Provides counseling and education to patients and families regarding healthcare needs.
  • Completes clinical notes: Documents patient progress with accurate and timely clinical notes.
  • Assists with procedures: Supports RNs and physicians in performing specialized procedures and duties.
  • Supports plan of care: Assists the RN in executing the patient’s plan of care.
  • Teaches self-care: Educates and supports patients in learning self-care techniques.
  • Adheres to nursing standards: Follows established nursing practices and communicates effectively within the healthcare team.

Required Qualification:

  • Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as a LPN/LVN in the state(s) in which he/she practices.
  • Must have a Valid CPR Certification.
  • Must have a Valid HIV/AIDS Course.
  • Must have Proof of a Valid Driver's License.
  • Must have Valid Auto Insurance.
  • Must Pass a Drug Screening.

Preferred Qualifications:

  • Three years (3) of experience working as an LPN in a home healthcare setting.

Salary/Hourly Rate:

LPN: $28.00/Hourly

6-13 weeks contracted in Altoona

Hours:

3pm-11pm

11pm-7am

Working Conditions:

  • Able to constantly travel within geographic area serviced by office from assignment to assignment.
  • Meets the applicable health requirements to provide patient care per applicable law or regulation.
  • Work is normally performed in patient’s home, or occupational settings.
  • Frequent exposure to communicable diseases, bloodborne pathogens and/or other potentially infectious or hazardous materials and situations that require following extensive safety precautions and may include the use of protective equipment.
  • Able to constantly stand, walk, bend, stoop, squat, kneel and reach freely.
  • Able to frequently lift, reposition, and transfer patients.
  • Able to constantly grasp with thumb in opposition to fingers or palm, e.g., manipulate pen, syringes, vials, knobs or objects.
