Job summary

Avera Behavioral Health Center-Sioux Falls is seeking a Licensed Practical Nurse (LPN) to join our adolescent and child units. You will provide focused nursing assessments, implement care plans, and collaborate with a multidisciplinary team to deliver high-quality patient care in a structured environment.

The role features rotating days/evenings/nights/weekends/holidays with a supportive team, opportunities for professional growth, and eligible benefits including PTO from day one and student

Qualifications

• Must hold an active LPN license in the state of practice.
• Ability to work the hours specified, with flexibility for rotating shifts.

Responsibilities

• Collects data and conducts focused nursing assessments.
• Participates with the health care team in developing and modifying patient-centered care plans.

A Brief Overview

The LPN practices under the supervision of a registered nurse, advanced practice registered nurse, licensed physician, or other health care provider. The LPN organizes and directs those aspects of care for which he/she is accountable; communicating and coordinating care with well-defined nursing diagnoses, including all health team members.

What You Will Do

• Collects data and conducts a focused nursing assessment of the health status of a patient.
• Participates with other health care providers in the development and modification of the patient-centered health care plan.
• Implements nursing interventions within a patient-centered health care plan.
• Assists in the evaluation of responses to interventions.
• Provides for the maintenance of safe and effective nursing care rendered directly or indirectly.
• Advocates for the best interest of the patient.
• Communicates and collaborates with patients and members of the health care team.
• Assists with health counseling and teaching.
• Delegates and assigns nursing interventions to implement the plan of care.
