Altra Home Health in Avon is seeking a dedicated Licensed Practical Nurse (LPN) to deliver in-home care to pediatric and adult patients with complex medical needs. You will provide G-tube feedings, wound care, trach/vent support, and education for patients and families in a home setting.

You’ll join a collaborative team, with flexible shifts ranging 4–12 hours. This role requires an active Indiana LPN license, CPR certification, and strong bedside judgment.

Job Summary

Altra Home Health is looking for a caring and skilled Licensed Practical Nurse (LPN) to become part of our team. In this rewarding position, you'll deliver personalized, top-notch care to pediatric and adult patients in the comfort of their own homes. Many of our patients face complex medical challenges, including gastrostomy tubes (G-tubes), cerebral palsy, chronic conditions, and occasionally tracheostomy or ventilator support. You'll be instrumental in fostering safety, comfort, and quality of life—empowering patients to receive the care they need right where they feel most comfortable: home.

• Experience with G-tube, trach, or vent care is a plus (training provided).

• Current CPR certification.
