Altra Home Health is seeking a caring Licensed Practical Nurse (LPN) to join our home-based care team in Avon. You will deliver personalized nursing care to pediatric and adult patients in their homes, with focus on safety, comfort, and quality of life.

Shifts range from 4 to 12 hours with flexible scheduling, including day, evening and weekend options. G-tube, trach, or ventilator care experience is a plus and training is provided.

Qualifications

• Active LPN license (Indiana or compact)
• 1–2 years of clinical home health nursing experience preferred
• G-tube, trach, or vent care experience is a plus (training provided)
• Current CPR certification
• Strong critical thinking and independent decision-making

• Reliable transportation and valid driver's license

Responsibilities

• Perform head-to-toe assessments and implement personalized care plans.

Job Summary

Altra Home Health is looking for a caring and skilled Licensed Practical Nurse (LPN) to become part of our team. In this rewarding position, you'll deliver personalized, top-notch care to pediatric and adult patients in the comfort of their own homes. Many of our patients face complex medical challenges, including gastrostomy tubes (G-tubes), cerebral palsy, chronic conditions, and occasionally tracheostomy or ventilator support. You'll be instrumental in fostering safety, comfort, and quality of life-empowering patients to receive the care they need right where they feel most comfortable: home.

Responsibilities

• Conduct comprehensive head-to-toe assessments and carry out personalized care plans.
• Deliver hands-on care such as G-tube feedings, medication administration, wound care, trach care, and ventilator support.
• Approach every patient interaction with professionalism and genuine empathy.
• Guide and empower patients and caregivers with education on disease management, emergency response, and therapeutic techniques.
• Partner closely with physicians, therapists, and fellow team members to deliver the best possible care.
• Keep accurate, timely documentation of all care activities in line with agency and state standards.
• Uphold the highest standards of safety, infection control, and patient privacy.

Shift Information

Qualifications
