Job summary

Senior Lifestyle in Ohio is seeking a Part-time Licensed Practical Nurse to provide licensed nursing services to residents in accordance with state and federal regulations and company guidelines. You will collaborate with the care team, perform admissions and assessments, and contribute to care planning and documentation.

The ideal candidate holds an active LPN license in Ohio, CPR certification, and at least 1 year of senior care experience.

Qualifications

• Training from an accredited licensing or vocational nursing program.
• Area of Study: General nursing.
• Years of Experience: 1+ years.
• Type of Experience: Senior care or related field.
• Special Certifications: LPN license current in Ohio and CPR certified.
• Language Skills: Proficient in English to perform duties and emergencies.

Responsibilities

• Administer medications per Federal & State regulations and Senior Lifestyle guidelines.
• May handle admissions, including assessments and documentation.
• Develop and complete acute and long-term care plans or service plans.
• Track clinical information (e.g., wounds, infection control) as directed.
• Provide health-related consultations within the community.
• Notify physicians/families of health changes with proper documentation.
• Respond promptly to emergencies and coordinate with staff.
• Maintain communication with all departments and staff.
• Provide educational meetings for residents and staff.
• Maintain confidentiality of resident information and records.
• May handle discharges, assessments, and clinical documentation as needed.

Job Description

• Part-time
• Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.
• May be responsible for the admissions of new residents to include all admission assessments and documentation.
• Responsible for completing acute and long-term care plans or service plans.
• May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.
• Provide general consultation regarding health-related concerns within the community.
• Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
• Respond quickly to all emergencies.
• Maintain close communication with all departments and staff members.
• Provide educational meetings for residents and staff.
• Maintain confidentiality of residents’ and prospective residents’ information.
• May be responsible for discharges, assessments, and clinical documentation as needed.

Qualifications

Level of Formal Education: Training from an accredited licenses or vocational nursing program

Area of Study: General nursing

Years of Experience: 1+ years

Type of Experience: Senior care or related field

Special Certifications: Must have a Licensed Practical Nurse’s License current in the state in which they are practicing. Also, needs to be CPR Certified

Language Skills: Acceptable proficiency in English to perform job duties, communicate with supervisor and other employees, and respond and communicate to emergency situations.

Technical Competencies: Basic computer skills
