Summary of Primary Function/General Purpose of Position

In collaboration with the interdisciplinary team and under the supervision of the professional Registered Nurse, the Licensed Practical Nurse (LPN) provides skilled nursing care and administers medications to patients.

Essential Job Functions
  1. Under the direction of a Licensed Professional (e.g., Physician, Physician Assistant, Nurse Practitioner, or Registered Nurse), participates in the nursing process including data collection, planning, implementation, and evaluation of the patient’s plan of care, accurately and timely, collaborating with other healthcare team members.
  2. Provides direct nursing care and assists with patient care at the direction of a Registered Nurse, Licensed Physician, Physician Assistant, or Nurse Practitioner.
  3. Contributes to the care plan with input from patients, families, and healthcare team members.
  4. Performs patient data collection and observation, considering patient’s age, condition severity, and needs, coordinating resources and referrals.
  5. Documents objective and subjective data, patient care activities, and observations following hospital guidelines.
  6. Implements patient care in a safe, knowledgeable, consistent, and continuous manner.
  7. Administers medications and treatments within scope prescribed by licensed providers using appropriate equipment.
  8. Performs phlebotomy services.
  9. Evaluates patient responses to interventions, involving the patient and family, and communicates findings to the healthcare team.
  10. Recognizes and reports significant changes in patient condition, contributing to plan revisions.
  11. Demonstrates sensitivity to diverse patient populations.
  12. Adheres to safety policies, recognizes potential safety issues, and reports them.
  13. Delegates tasks according to State Board of Nursing guidelines.
  14. Accepts accountability for delivering optimal patient outcomes.
  15. Complies with HIPAA and OSHA policies and practices infection control procedures.

Licensing/Certification
  • Current LPN license from the state of employment (required)
  • BLS from American Heart Association (required)
  • ACLS, PALS, NRP (may be required for specialty areas)
Education

None required

Work Experience

Preferred: 1 year of clinical patient care experience in an acute care, long-term care, or physician practice setting.

Training
  • EPIC EHR training (preferred)
  • Basic Arrhythmia course with passing exam (required for Telemetry units)

Working Conditions

May include high stress, physical altercations, exposure to weather, noise, hazardous substances, blood, and bodily fluids. Use of protective equipment and infection control policies is required.

Physical Requirements

Includes lifting, carrying, pushing, pulling, stooping, crawling, climbing, balancing, bending, sitting, walking, and standing, with specified weight limits and percentages.

Additional Physical Requirements/Hazards
  • Dexterity, shift work, maneuvering patients, hearing alarms, reaching, repetitive movements, vision, and chemical exposures.
Patient Population

Capable of providing age-appropriate care for neonates, infants, children, adolescents, adults, and geriatrics, with knowledge of growth, development, and assessment skills.
