Lifepoint Health is hiring an LPN for the Acute Rehab unit in Ottumwa, IA. This full-time night shift role (7:00 PM–7:00 AM) involves direct patient care, wound care, medication administration, and collaborating with RNs to support care plans.

Qualifications

• Graduate of an accredited LPN program with current Iowa LPN license or a compact-state license.
• Basic Life Support (BLS) certification.

Responsibilities

• Performs plan of care interventions, including specimen collection and wound care.
• Administers medication per scope of practice.
• Documents patient care given.
• Assists RN with development of plan of care.
• Assists patients with activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
• Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.
• Works as an advocate for the physical and emotional well-being of the patient.
• Adherence to all core measure initiatives, patient safety goals, and NQF Safe Practices.

Position Summary

The LPN works under the supervision of the department Director and Manager, and the registered nurse.

• Performs plan of care interventions, including specimen collection and wound care.


• Administers medication per scope of practice.


• Documents patient care given.


• Assists RN with development of plan of care.


• Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.


• Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.

Minimum Qualifications

• Graduate of an accredited registered nursing program required with a current unencumbered Licensed Practical Nurse License in Iowa or from a Compact State.


• Basic Life Support (BLS)

Preferred Qualifications

• At least 1 year of LPN experience (acute care experience preferred)
