Job summary

Cayuga Health is seeking a Licensed Practical Nurse for the Seneca View Skilled Nursing Facility, Montour Falls, NY. You will assist with assessment, planning, implementation and evaluation of patient care during evening shifts, collaborating with the RN and care team to deliver safe, person-centered care.

Requirements include graduation from a nursing program, a current NYS LPN license, and CPR/BLS. A sign-on bonus of $7,500 is offered plus benefits and on-site support.

The primary functions of an Licensed Practical Nurse (LPN) in assisting with assessment, planning, implementing and evaluating the care of all patients, regardless of age, during his/her shift. Responsible for meeting the established standards of skilled nursing practice. The Licensed Practical Nurse collaborates with the care team, participates in interdisciplinary care, and advocates for patients. Effectively provides Person Centered care and services.

Roles and Responsibilities:

• Administers medications and treatments under the supervision of a Registered Professional Nurse (RN) and in accordance with facility policies and procedures.
• Monitors and records patient/resident vital signs and conditions, promptly reporting changes or pertinent observations to the RN and appropriate nursing personnel.
• Provides ethical, professional, and patient-centered care within scope of practice while advocating for patients/residents.
• Maintains clinical competency through required education and training; coordinates care effectively with the interdisciplinary team.
• Completes accurate and thorough documentation in the EMR/Point Click Care system and actively participates in unit councils, staff meetings, and other team activities.

Required Skills and Experience:

• Graduate of a Nursing Program
• Current NYS LPN License
• CPR/BLS

Preferred Skills and Experience:

• Prior patient care and customer service experience

Physical Requirements:

• May require lifting (up to 50 lbs.), may require sitting or standing for long periods. Must be able to physically operate the equipment required for the job

Location, Travel and Shift Requirements:

• Onsite – Seneca View Skilled Nursing Facility – 220 Steuben St Montour Falls, NY 14865
• Shift – (Evenings) 2:30 PM - 11:00 PM
  • This Role requires work every other weekend

Pay Range Disclosure:

• Standard Rate of $26.15 - $37.00 per hour plus benefits
• Sign On Bonus - $7,500.00
