Job summary

Claremore Skilled Nursing & Therapy is seeking a dedicated Licensed Practical Nurse (LPN) to join a supportive, well-structured team. You will administer medications, monitor residents, and coordinate care with families and physicians in a compassionate facility environment.

The role offers 12-hour rotating shifts (6am-6pm schedule with off days), competitive pay, and comprehensive benefits for full-time staff.

Qualifications

• Licensed practical nurse responsibilities include administering medications and treatments per nursing standards.
• Observe residents and report changes to the team.
• Record vital signs and document patient status.
• Communicate with families and physicians about patient care.

Responsibilities

• Obtain and administer prescribed medications and treatment in accordance with approved nursing techniques.
• Observe residents and record significant conditions and reactions; notify changes.
• Take vital signs to detect deviations and assess condition.
• Keep families and physicians informed of changes in health and medications.
• Modify care plans and ADLs in response to condition changes.
• Provide wound care and prepare/ administer injections as needed.
• Maintain accurate nursing documentation and ensure chart accuracy.

Available Shift (With Shift Differentials): 6am-6am (You will have 3-4 days off per week with 12-hour rotating shifts.

• Week 1: Mon, Tue, Sat*, Sun*
• Week 2: Wed, Thu, Fri

Responsibilities and Purpose

• Obtain and administer prescribed medications and treatment in accordance with approved nursing techniques. Ensure residents’ chart is current
• Observe residents and record significant conditions and reactions. Notify appropriate parties of residents’ change in condition and reaction to drugs, treatments and significant incidents.
• Take vital signs to detect deviations from normal and assess condition of residents.
• Keep residents’ family and physician informed of any changes in resident behavior, medications or conditions of health.
• Modify residents’ care plan, care directives, Activities of Daily Living (ADL) and Nurse Aide flow sheets in conjunction with residents’ changes in condition.
• Prepare and administer injections and/or enemas.
• Collect samples and monitor catheters.
• Provide wound care.
