Claremore Skilled Nursing & Therapy in Claremore, OK is seeking a Licensed Practical Nurse (LPN) to join a supportive, well-structured team. You will administer medications, monitor residents, update charts, and collaborate with physicians and families to ensure high-quality care.

Formación

• Current Oklahoma Licensed Practical Nurse (LPN) license.
• 1+ years of experience in a skilled nursing or long-term care environment.

Responsabilidades

• Obtain and administer prescribed medications and treatments per nursing techniques and keep charts current.
• Observe residents, note significant changes, and inform physicians, families, or staff as needed.
• Take vital signs and assess residents' condition regularly.
• Inform family and physician of changes in health, medications, or behavior.
• Modify care plans and ADL/Nurse Aide flow sheets as conditions change.
• Prepare and administer injections or enemas as directed.
• Collect samples, monitor catheters, and provide wound care.

Available Shift (With Shift Differentials): 6am-6pm (You will have 3-4 days off per week with 12-hour rotating shifts)

• Week 1: Mon, Tue, Sat*, Sun*
• Week 2: Wed, Thu, Fri

Responsibilities and Purpose

• Obtain and administer prescribed medications and treatment in accordance with approved nursing techniques. Ensure residents’ chart is current
• Observe residents and record significant conditions and reactions. Notify appropriate parties of residents’ change in condition and reaction to drugs, treatments and significant incidents.
• Take vital signs to detect deviations from normal and assess condition of residents.
• Keep residents’ family and physician informed of any changes in resident behavior, medications or conditions of health.
• Modify residents’ care plan, care directives, Activities of Daily Living (ADL) and Nurse Aide flow sheets in conjunction with residents’ changes in condition.
• Prepare and administer injections and/or enemas.
• Collect samples and monitor catheters.
• Provide wound care.

Required Experience and Education

• Current and unencumbered Oklahoma Licensed Practical Nursing (LPN)License.
• 1+ years of experience in a Skilled Nursing or Long Term Care environment.
