Job summary

AccentCare Forest, MS is seeking an LVN/LPN to provide direct patient care under RN supervision for pediatric to geriatric patients across Newton and Jasper counties. This role uses a per-point compensation structure, with a published annual estimate of $49,000–$54,000.

Schedule is Mon-Fri 8:00 a.m.–4:30 p.m., with occasional on-call visits. Requirements include graduation from an accredited program, valid LVN/LPN license in the state, CPR certification, and a valid driver’s license to travel

Qualifications

• Graduate of an accredited practical or vocational nursing program
• Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation
• One year of LVN/LPN work experience
• Ability to provide nursing visits according to physician treatment and maintain medical records
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver who can travel to all business locations

Responsibilities

• As a LVN/LPN, you will: Performs technical nursing functions and direct patient care under RN supervision
• Promotes and maintains optimal client care under RN supervision
• Maintains a safe patient environment
• Identifies and communicates significant abnormalities in patient data to case manager and home health team
• Administers prescribed medications and treatments, assesses medicines for interactions or adverse effects
• Re-evaluates client's nursing needs and reports findings to Case Manager
• Counsels and instructs client and family in meeting nursing needs
• Prepares clinical and progress notes and submits documentation per policy
• Documentation on patients chart correlates with nursing care plan
• Communicates with home care team and coordinates services

The LVN/LPN observes and documents the patient's medical progress and nursing status changes under the direction of a registered nurse. This position provides direct patient care to pediatric, adolescent, adult and geriatric patients.

• Administers prescribed medications and treatments as permitted by law. Assesses all medicines a client may be taking to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, contraindicated medications and reports any problems to the physician.
• Re-evaluates client's nursing needs and reports findings to Case Manager.
• Counsels and instructs the client and family in meeting nursing and related needs.
• Prepares clinical and progress notes.
• Submits all documentation in a timely manner in accordance with agency policy.
• Documentation on patients chart correlates with nursing care plan.
• Communicates frequently and works closely with other home care team members.
• Assists in coordinating services provided to client
