Job summary

Ocean Meadow Senior Living in Clinton, CT is seeking an LPN to administer medications and support residents with memory challenges. The role involves admissions, care plan implementation, and coordination with physicians and families to ensure high-quality care.

Eligibility includes a current LPN license with CPR certification and 1–2 years in nursing, preferably with elderly populations. The position offers full-time, part-time, or per diem schedules and a comprehensive benefits package.

Qualifications

• Current state license as an LPN and CPR certification.
• Graduate of an approved LPN program (per state requirements).
• Minimum of 1 to 2 years of experience working as a nurse preferred.
• Previous experience working with the elderly in a paid or volunteer position is highly desirable.

Responsibilities

• Responsible for medication administration for the residents in compliance with Federal & State regulations.
• Assist with admissions of new residents, including assessments and documentation.
• Assist with the process of implementing acute and long-term care plans or service plans and/or the collection of data.
• Responsible for clinical system tracking under the director's direction.
• Provide general consultation regarding health-related concerns within the community.
• Notify physicians and/or family members of any change in the resident's health and provide proper documentation.
• Respond quickly to all emergencies.
• Maintain close communication with all departments and staff members.
• Provide educational meetings for residents and staff.
• Understand the role of the Safety & Disaster Plan.
• Maintain confidentiality of residents' and prospective residents' information.
• Attend all required training, in-service, and staff meetings.
• Maintain a safe working environment through prevention of accidents, preservation of equipment, and safe practices.
• Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers.
• Perform other duties as assigned.
