Job summary

Oak Knoll Health and Rehabilitation in Birmingham, AL is seeking a licensed practical nurse (LPN) to provide direct nursing care to residents in a long-term care and rehabilitation setting. You will administer medications, assist with rehabilitation plans, and collaborate with RNs and physicians to ensure the highest standards of care.

This full-time evening shift (3 PM–11 PM) offers a $10,000 sign-on bonus, competitive hourly pay of $25.00–$32.00 based on experience, and full benefits including

Qualifications

• Active LPN license.
• Strong clinical skills including med administration and wound care.
• Excellent communication and interpersonal skills.

• Compassionate, patient-focused approach to care.
• CPR/BLS certification required.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents' health status including vital signs; report changes to RN/physician.
• Assist with activities of daily living (ADLs) as needed.
• Collaborate with rehabilitation team to support recovery goals (PT/OT/SLT).
• Maintain accurate documentation per facility policies and regulations.
• Provide emotional support and education to residents and their families.
• Ensure a safe, clean environment with infection control practices.

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. In this role, you will provide direct nursing care to residents, assist with rehabilitation plans, and collaborate with RNs, physicians, and interdisciplinary team members to ensure the highest standards of care in our long-term care and rehabilitation setting.

Qualifications:

1. Current and valid LPN license


2. Strong clinical skills, including medication administration and wound care.


3. Excellent communication and interpersonal skills.


4. Ability to work effectively in a team-oriented environment.


5. Compassionate, patient-focused approach to care.


6. BLS/CPR certification required.

Responsibilities:

1. Administer medications and treatments as prescribed by physicians.

3. Assist with activities of daily living (ADLs), including bathing, dressing, and feeding, as needed.


4. Collaborate with the rehabilitation team to support residents’ recovery goals, including physical, occupational, and speech therapy plans.


5. Maintain accurate and timely documentation in accordance with facility policies and state/federal regulations.


6. Provide emotional support and education to residents and their families.


7. Ensure a safe and clean environment, adhering to infection control protocols.
