Job summary

Northway Health and Rehabilitation in Birmingham, AL is seeking a skilled Licensed Practical Nurse (LPN) to join our long-term care and rehabilitation team. You will provide direct nursing care to residents, administer medications, and collaborate with RNs, physicians, and therapists to support recovery goals.

Qualifications include an active LPN license and BLS/CPR certification. We offer full-time evening shifts (3:00 PM – 11:00 PM), shift differential pay, comprehensive health and vision

Qualifications

• Strong clinical skills including medication administration and wound care.
• Excellent communication and teamwork.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor and document residents’ health status and vital signs.
• Assist with activities of daily living (ADLs) such as bathing, dressing, and feeding.
• Collaborate with the rehabilitation team to support recovery goals.
• Maintain accurate documentation per facility policies and state/federal regulations.
• Provide emotional support and education to residents and their families.
• Ensure a safe and clean environment with infection control.

Position Overview: Licensed Practical Nurse ( LPN )

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. In this role, you will provide direct nursing care to residents, assist with rehabilitation plans, and collaborate with RNs, physicians, and interdisciplinary team members to ensure the highest standards of care in our long-term care and rehabilitation setting.

Qualifications:

1. Current and valid LPN license
2. Strong clinical skills, including medication administration and wound care.
3. Excellent communication and interpersonal skills.
4. Ability to work effectively in a team-oriented environment.
5. Compassionate, patient-focused approach to care.
6. BLS/CPR certification required.

Responsibilities:

1. Administer medications and treatments as prescribed by physicians.
2. Monitor and document residents’ health status, including vital signs, and report changes to the supervising RN or physician.
3. Assist with activities of daily living (ADLs), including bathing, dressing, and feeding, as needed.
4. Collaborate with the rehabilitation team to support residents’ recovery goals, including physical, occupational, and speech therapy plans.
5. Maintain accurate and timely documentation in accordance with facility policies and state/federal regulations.
6. Provide emotional support and education to residents and their families.
7. Ensure a safe and clean environment, adhering to infection control protocols.
