Excelsior Care Group seeks a Licensed Practical Nurse (LPN) for Throgs Neck Rehabilitation and Nursing in the Bronx, NY. This full-time role covers all shifts and offers competitive hourly pay up to $45.

You will administer medications, follow care plans, and collaborate with nursing leadership to deliver high-quality resident care. A current NY LPN license and prior SNF experience are preferred.

Qualifications

• Valid LPN license in the State of New York
• Experience in a Skilled Nursing Facility preferred
• Passion for improving residents’ quality of life
• Strong commitment to safety, professionalism, and teamwork

• Administer medications and treatments in accordance with federal, state, local, and facility regulations
• Collaborate with the Director of Nursing and Assistant Director of Nursing to ensure high-quality resident care
• Order necessary medications, supplies, and equipment
• Follow individualized care plans tailored to each resident’s needs
• Perform wound care and related treatments
• Document observations, interventions, and outcomes accurately and thoroughly
• Maintain confidentiality in compliance with HIPAA, respecting the rights of residents, families, and staff
