Job summary

Aveanna Healthcare is seeking an LPN/LVN to provide in-home skilled nursing care. You will assess patients, implement care plans, and document interventions using the in-home tablet. Flexible scheduling with full-time, part-time, or PRN shifts, including days, nights, and weekends.

Qualifications

• Must have and maintain an active LPN/LVN license in the state of practice.
• Current CPR certification with hands-on component.
• TB skin test current within last 12 months.
• Six months of hands-on nursing experience preferred but not required.

Responsibilities

• Provide skilled nursing care and assessment to patients in their homes.
• Develop and update care plans in collaboration with clinicians.
• Document care delivery using the tablet in each patient’s home.
• Collaborate with the care team to achieve optimal clinical outcomes.

Qualifications

• Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
• Compact licenses must be transferred to your state of residence within 90 days
• Current CPR certification (with hands-on component)- Aveanna can assist in obtaining this requirement after hire, if necessary.
• TB skin test (current within last 12 months)
• Six months prior hands-on nursing experience preferred but not required
• Must have reliable transportation
