Job summary

Amivie, LLC in Woonsocket, RI is seeking a compassionate Licensed Practical Nurse (LPN) to join our home care team. The role focuses on patient care, developing individualized plans, and advocating for client needs, with both part-time and full-time options and a bi-weekly rotating schedule.

Typical shifts are 10:30 AM to 5:30 PM, with days Tue/Fri in Week 1 and Wed/Thu/Fri in Week 2, ensuring consistent patient support and care continuity.

Qualifications

• Current Rhode Island LPN license is required.
• Experience in direct care services or related field is preferred.
• ICF/MR or experience with persons with disabilities is a plus.

Responsibilities

• Document medical changes in patient charts as required and contact proper people as necessary.
• Administer and monitor medication administration and treatments as necessary.
• Provide and assist in training for individuals and direct care staff as needed.
• Ensure prescription instructions are followed as prescribed by proper authority.
• Make recommendations that may improve an individual's development.
• Comply with Corporate policies, HIPAA guidelines, DHHS rules, and Medicaid state and federal guidelines.
• Comply with Corporate Compliance Program policies and procedures.
