What You’ll Do

• Provide skilled nursing care to clients in their homes under the direction of a Registered Nurse.
• Conduct timely and thorough assessments and deliver treatments based on physician orders and care plans.
• Educate clients and families on health conditions and self‑care.
• Document all care activities in accordance with agency protocols and EMR requirements.
• Respond to client needs during assigned on‑call shifts and per diem requests with professionalism and empathy.
• Collaborate with interdisciplinary teams to ensure continuity and quality of care.

Who You Are

• Licensed Practical Nurse (LPN) in the state of Connecticut.
• 1+ year of nursing experience (home health preferred).
• Bilingual (Spanish/English) highly preferred to support communication with our diverse patient population.

• Comfortable working independently in home settings.
