Job summary

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our care team. The role involves assessing referred patients, developing treatment plans, and delivering direct therapy in a rehabilitation setting.

The SLP will document progress, educate patients and families, and collaborate with an interdisciplinary team. TN licensure is required; prior long-term care or geriatric experience is preferred; sign-on bonus available.

Qualifications

• Graduated from an accredited Speech-Language Pathology program.
• Valid TN license and registration.
• Experience in long-term care or geriatric settings preferred.
• Excellent documentation and reporting skills.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Develop and administer a treatment program with specific goals.
• Document and record patient condition; educate patients and families.
• Collaborate with all disciplines to provide patient-centered care.
• Complete evaluations, progress notes, recertifications and discharge documentation.

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team.

Speech Language Pathologist (SLP) Responsibilities Include:

The Speech Language Pathologist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Speech Language Pathologist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Speech Language Pathologist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Speech Language Pathologist (SLP) Qualifications/Requirements:

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of TN
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
