Chatham Hills Subacute Care Center is seeking a Licensed Practical Nurse (LPN) for day shift in a skilled nursing facility. You will provide patient care in the SNF and Long Term Care units, administer medications including IVs, monitor vital signs, and educate residents and families. You will coordinate with physicians and supervise CNAs to ensure quality care.

Qualifications

• Current LPN license in good standing.
• Experience in SNF/long-term care preferred.
