Job summary

Green Hill Senior Living & Rehabilitation in West Orange, NJ invites an LPN to join our compassionate nursing team. You will supervise CNAs, provide individualized care per each resident’s plan, and assist with admissions, transfers, and discharges.

Administer medications, explain procedures to residents, monitor vital signs, and maintain accurate nursing records to ensure continuity of care and high-quality outcomes.

Qualifications

• Active license as an LPN in New Jersey.
• Excellent communication and interpersonal skills.
• Ability to work independently or as part of a team.

Responsibilities

• Supervise, coordinate, and direct the work of nursing assistants (CNAs).
• Provide individualized nursing care to residents according to each resident’s plan of care.
• Assist in the admission, transfer, and discharge of patients.
• Administers prescribed medications and treatments as ordered by the physician.
• Explains procedures and treatments ordered to gain patients’ cooperation and allay apprehension.
• Observe vital signs to detect deviations from normal and gauge progress.
• Maintain patients’ medical records on nursing observations and actions taken.
• Record nursing needs of patients on care plans to ensure continuity of care.

What You'll Do

• Observes emotional stability of patients, expresses interest in their progress, and prepares them for continuing care after discharge.
• Perform other duties as required to provide exceptional care to each resident.

What You Need

• Active, unencumbered licensure as a Licensed Practical Nurse (LPN) in New Jersey.
• Excellent communication and interpersonal skills.
• Highly organized and detail oriented.
• Ability to work independently or as part of a team.
