AuthentiCare Health Centers in Massachusetts is seeking a compassionate Licensed Practical Nurse (LPN) to join our caring family in our skilled nursing facilities. You will administer medications, monitor residents, and support a resident-centered care plan in a memory care, sub-acute and long-term care setting.

We offer a $5,000 sign-on bonus, paid weekly and a benefits package including health insurance, 401k and tuition reimbursement.

Qualifications

• Must hold MA RN or LPN license.
• Experience in long-term care or memory care preferred.
• Strong clinical skills and resident-centered care mindset.
• Excellent communication and documentation abilities.

Responsibilities

• Assess resident health problems and needs, develop and implement nursing care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor resident progress and respond to changes in condition.
• Collaborate with multidisciplinary teams to provide comprehensive care.
• Educate residents and families regarding health conditions and discharge.
• Maintain accurate resident records complying with state/federal regulations and HIPAA.
• Participate in case management to ensure continuity of care.
• Support in specialized areas such as PT, OT, SUD, Pain Management.

RESPONSIBILITIES

Including, but not limited to:

• Maintain accurate resident records and documentation in compliance with state and federal regulations, while ensuring compliance with HIPAA regulations.
• Participate in case management activities to ensure continuity of care.
• Provide support in specialized areas such as PT, OT, SUD, Pain Management, etc.

SKILLS

• Registered or Licensed Nurse in the state
• Medication administration
• EMR: PCC (Point Click Care)
• Proficiency in memory care techniques and protocols.
• Knowledge of MDS (Minimum Data Set) assessments for long-term care facilities.
• Familiarity with operations in Skilled Nursing, Sub-Acute, and Dementia Units.
• Strong background in case management practices.
• Ability to work effectively in a medical office environment.
• Excellent interpersonal skills for effective communication with patients, families, and healthcare teams.
• Strong organizational skills to manage multiple tasks efficiently.
